CONGRESS PRE-REGISTRATIONS RULES
Important: by registering to this congress, you automatically accept and agree with the pre-registration rules.

 

DELEGATES
Registration fee entitles the delegate to attend all Scientific and Poster Sessions, Opening Ceremony, Welcome Cocktail, Industrial Exhibition, and to receive the congress kit (including Abstract Book and Certificate of Attendance) and coffee break tickets.
Each delegate must fill in a separate form. For additional forms please make photocopies, download and print it or contact the ERA-EDTA Congress Office.

ACCOMPANYING GUESTS
• Accompanying guests’ registration fee includes admission to Opening Ceremony, Welcome Cocktail and a half-day sight-seeing tour of Istanbul.
• Accompanying guests are not entitled to attend any of the other Congress initiatives (i.e. Industrial Exhibition, Session and Poster Halls).
• Accompanying guests’ registration is accepted only in connection with delegate’s registration.
Children under the age of 12 cannot be registered as accompanying guests. They can be admitted to social functions, only if accompanyied by regularly registered adults, and their number must be limited to one child per registered adult. They will not be permitted in the exhibition/poster/session halls at any time.

DELIVERY
• In order to avoid any misplacing, please make sure that the Pre-Registration form is sent to the correct address/fax below:
ERA-EDTA Congress Office, Via Spolverini 2, 43100 Parma, Italy, Fax: +39-0521-959242.
• Make sure that you don’t send your form to Figur or to the President/Secretary’s Offices in Turkey.
• Fill in the form and send it to the ERA-EDTA Congress Office, together with the Payment (credit card details, cheque or copy of transfer receipt issued by the bank).
• Do not forget to send the Pre-Registration form completely filled in, when you send the payment by bank transfer: without it we cannot insert you in the pre-registered delegates’ list.
• In order to benefit of the Pre-Registration fees, forms and payments,all must be received within the deadlines. Submission of form without payment within deadlines, does not entitle to benefit of the reduced fees later.
• The mentioned deadlines regard the receipt date and not the postal date.

PAYMENT
• Pre-registration forms received without payment or proof of payment will not be processed.
• Please clearly indicate your name, address and ERA-EDTA on money orders.
• If you send your Pre-Registration form by fax with a credit card payment, do not also mail a copy, since it may result in a duplicate registration and charged twice.
• Credit cards might be charged in Swiss Francs.

• Always keep a copy of proof of payment, or fax confirmation, for your records!

MEMBERSHIP
• Membership and payment of ERA-EDTA membership fees will be verified for all individuals who indicate they are ERA-EDTA members, thus make sure that you are a current ERA-EDTA member and have regularly paid your membership fees.
• If you wish to pre-register as member and you are not one, contact the ERA-EDTA Membership Office to become a member before pre-registering.

CONFIRMATION
• A confirmation letter will be sent by post or e-mail to regularly pre-registered delegates after the early deadline for those who have pre-registered within February 7, 2005 and after the late deadline for those who have pre-registered within May 2, 2005.

RECEIPTS
• Receipts or written confirmations of payment are not automatically sent. If you wish to receive a receipt of the payment, clearly state this when sending the form and payment.
• Due to the fact that ERA-EDTA is a registered English Charity, invoices cannot be issued.

BLANK GROUP REGISTRATIONS
(companies only)
Companies that wish to pre-register a group of more than 15 delegates with the Blank List method, must contact the ERA-EDTA Congress Office for all details.
Deadline for bookings and payments is February 7, 2005. After February 7, 2005 normal pre-registration procedure is applied to all, groups included.

NO NAME CHANGES ARE POSSIBLE

CANCELLATION POLICY
Before May 2, 2005 75% of the amount paid will be reimbursed.
After May 2, 2005 no refund of the amount paid.
• All cancellations must be done in writing and must be received (not sent) within May 2, 2005.
• All refunds, if any, will be done after the Congress and will be in Euro currency only.

ON-SITE REGISTRATION
(please read this carefully after May 2, 2005)
• On-site forms will be available near the New Registration desk (starting June 4, 2005 from 9.00 to 18.00 hours), located in the Registration Hall of the ICEC in Istanbul: you must fill in the on-site form and take it, together with the payment to the above mentioned desks.
• It will be possible to pay by bank cheque (we don’t accept personal cheques), bank transfer (in which case a copy of the original bank transfer receipt issued by the bank must be handed in together with the form), cash (Euro only) and credit card (Visa and Eurocard/Mastercard). We don’t accept Visa Electron, American Express or Diners card.

• The On-Site Registration fees will be the following:

* available only on-site:to qualify for this fee, you must personally hand in a photocopy of your identification picture document, show your original students card and give a signed official letter from your mentor/university to proof your status. ERA-EDTA reserves the right to verify all declarations.
Companies or other subjects will not be allowed to register delegates at this fee.


 

 




 

 

• Please note that after May 2, 2005, you will not be included in the pre-registered file, even though the payment has been sent.
If the late pre-registration fee has been paid by bank transfer, the on-site fee must be assessed and the difference must be paid directly in Istanbul.
